I wish to have my child participate in activities at Christian Assembly of God. I understand that there are risks inherent in physical activities and possible allergies, and I accept full responsibility for my child's health and safety during their participation while at Christian Assembly of God. I agree that my child's participation is fully voluntary and that I will not hold either Christian Assembly of God or their volunteers responsible for any harm that may result from my child's participation during these activities.
